All UF students must submit the Certification of Course Approval form in order to complete the registration process for a UF Flexible Learning Course. This form requires the signature of your college dean’s office or your academic advisor. The signed form can be submitted to our office by email, fax, or mail.

You will not be able to begin your new course(s) until we have received this signed approval form.

If you are a Florida resident, you must send us proof of residency. Please provide two or more documents to support your claim along with a Residency Declaration for Tuition Purposes form to avoid paying out-of-state fees. Since you selected “Yes”, please enter your account number in the above field. The Flexible Learning staff will apply your Florida Prepaid to your registration. In addition, your credit/debit card will be authorized for the full amount.

Once your plan number and registration are submitted, Florida Prepaid will be invoiced for the amount your plan covers, and your credit/debit card will be charged for any remaining balance.
